EXTRACT FROM A CRYONICIST'S WILL

WHEREAS, I, XXXXXXXXXX, of XXXXXXXXXX, XXXXXXXXXX, believe that the
experimental procedure of cryogenic preservation offers the possibility,
which is speculative, of eventual restoration of my human remains,
including at a minimum the remains of my brain and brain stem, to life
and health; and

WHEREAS, I desire that upon my legal death my human remains, to include
at a minimum the remains of my brain and brain stem, be preserved by the
experimental procedure of cryogenic preservation;

and

WHEREAS, I have executed separate instruments, entitled "Consent
to Cryopreservation," "Authorization of Anatomical Donation," and
"Cryopreservation Agreement," whereby I have arranged for the XXXXXXXXXX
Foundation, Inc., a XXXXXXXXXX Corporation with principal offices in
XXXXXXXXXX, XXXXXXXXXX, including any agent of the XXXXXXXXXX Foundation,
to undertake the cryogenic preservation of my human remains, to include
at a minimum the remains of my brain and brain stem, and for the
XXXXXXXXXX Foundation, its heirs, successors, or assigns to attempt, in
its or their sole discretion, to restore such remains, once cryogenically
preserved, to life and health.

NOW THEREFORE, I, XXXXXXXXXX, of XXXXXXXXXX, XXXXXXXXXX, being of lawful
age and of sound and disposing mind and memory, do hereby make, publish
and declare this to be my Last Will and Testament, to-wit:

FIRST:  I hereby revoke all other wills and codicils heretofore made
by me.

SECOND:  I authorize and direct that immediately upon my legal death my
human remains be delivered to the XXXXXXXXXX Foundation, including any
agent of the XXXXXXXXXX Foundation.  The phrase "agent of the XXXXXXXXXX
Foundation" means an agent, representative, contractor, employee,
volunteer, assistant, associate, physician, or other person authorized
to act on behalf of the XXXXXXXXXX Foundation.

THIRD:  I direct that no embalming or voluntary autopsy of my human
remains take place except on the direction of the XXXXXXXXXX Foundation,
including any agent of the XXXXXXXXXX Foundation.

FOURTH:  I authorize the XXXXXXXXXX Foundation, its heirs, successors, or
assigns (with all rights and responsibilities related to the cryogenic
preservation of my human remains also devolving upon the heirs,
successors, or assigns of the XXXXXXXXXX Foundation), including any agent
of the XXXXXXXXXX Foundation, to undertake the experimental procedure
of cryogenic preservation of my human remains, to include at a minimum
the remains of my brain and brain stem, and to attempt, in the sole
discretion of the XXXXXXXXXX Foundation, its heirs, successors, or
assigns, to restore such remains, once cryogenically preserved, to life
and health, all in accordance with the "Consent to Cryopreservation,"
"Authorization of Anatomical Donation," and "Cryopreservation Agreement"
executed by me.  I so authorize in full knowledge that such procedure is
inconsistent with traditional methods for disposition of human remains.

FIFTH:  I direct my Executor, named hereinafter, to exercise all powers
and defenses necessary to ensure speedy delivery of my human remains
to the XXXXXXXXXX Foundation, including any agent of the XXXXXXXXXX
Foundation, and to ensure that no embalming or voluntary autopsy of my
human remains takes place except on the direction of the XXXXXXXXXX
Foundation, including any agent of the XXXXXXXXXX Foundation.

SIXTH:  I authorize any person having my medical records to provide them
to the XXXXXXXXXX Foundation, including any agent of the XXXXXXXXXX
Foundation, and I direct my Executor and my family to provide whatever
assistance is requested by the XXXXXXXXXX Foundation, including any
agent of the XXXXXXXXXXX Foundation, to obtain such records.

SEVENTH:  I direct my Executor, named hereinafter, to pay death taxes
that may be assessed against my estate and all of my just debts,
including: a reasonable commission to my Executor for his services as
such; the costs and expenses that were incurred pursuant to the Advance
Medical Directive executed by me and that are not the responsibility of
the XXXXXXXXXX Foundation or a third party; and the costs and expenses
incurred by my Executor in carrying out or attempting to carry out the
provisions of this my Last Will and Testament.  I direct that no such
death taxes shall be apportioned or charged against any beneficiary
under this my Last Will and Testament.  I direct my Executor to use any
property in my estate (except for any property passing to the XXXXXXXXXX
Foundation through the Ninth Article of this my Last Will and Testament)
to the extent necessary to pay such just debts and death taxes.

EIGHTH:  I give, devise and bequeath the sum of two thousand dollars
($2,000) to the Life Extension Society, Inc., a Maryland corporation.

NINTH:  If and only if the XXXXXXXXXX Foundation, including any agent of
the XXXXXXXXXX Foundation, should undertake the experimental procedure of
cryogenic preservation of my human remains, to include at a minimum the
remains of my brain and brain stem, then I give, devise, and bequeath to
the XXXXXXXXXX Foundation the following property, including all powers of
appointment and other rights of disposition, to be used to pay the costs
and expenses of the experimental procedure of cryogenic preservation of
such remains, plus the costs and expenses of any attempt, in the sole
discretion of the XXXXXXXXXX Foundation, its heirs, successors, or
assigns, to restore such remains, once cryogenically preserved, to life
and health, all in accordance with the "Consent to Cryopreservation,"
"Authorization of Anatomical Donation," and "Cryopreservation Agreement"
executed by me:

TENTH:  To my wife, XXXXXXXXXX, should she survive me by at least thirty
days, I give, devise and bequeath all of the rest, residue and remainder
of my property of every kind and nature whatsoever, whether real,
personal, mixed or business, wheresoever situated, which I now own
or possess or may hereafter acquire, or to which I may be entitled,
including all powers of appointment and other rights of disposition.

NINETEENTH:  I acknowledge and accept that the experimental procedure of
cryogenic preservation is unproven and involves unforeseeable problems.
Accordingly, I hereby hold my Executor, the XXXXXXXXXX Foundation
(including any and all agents of the XXXXXXXXXX Foundation), and all
other persons acting pursuant to the Second, Third, and Fourth Articles
of this my last Will and Testament free from any liability arising from
acts or omissions pursuant thereto done in good faith.
