Certainly.  However, even with howlingly bad management, a big
organization takes a long time to die.  All a manager has to do is meet
minimal expectations, and s/he can stay in her/his little fiefdom
indefinitely.

This is even without Enron- style number- jiggering.  Consider office
politics -- the big egos are *very* good at showing why the failures are
all somebody else's fault.  When I was working for the Navy, the joke
after a major failure was "they fired a secretary or a draftsman".  For
some unknown reason, we had a shortage of secretaries and draftsmen ...
