What had been puzzling me was that gastric bypass requires a general, which
I gather is something you try to avoid when someone is in critical
care.  Once I had time to actually read email rather than just pass things
along it was clear that the gastric bypass would be later on.

I can't recall who it was at Philcon today who confirmed that it is a
gastric bypass; that Jack has to lose at least 100 pounds; and that his
condition has been upgraded to 'guarded'.
