What design >flaw? It's missing stick figure drawings on pages 219, 221 & 223. There's about six lines of blankness of those pages. The story is, I think, THe Shadow Knows. When Tor reissued the corrected version - and the copyright page still states First Edition / First Printing - they added a star like device next to the bar code, to make it easier to tell the editions apart as they defective ones came in.
