That was at a previous job...and no, none at the current one either.  We
just laid off half the QA staff (i.e. one guy).

Not usually, though we generally have a pretty good idea where we're going.
That again was a previous job...and there we didn't have much idea where we
were going, so we implemented the thing at least 6 times before I left...
