Regarding the use of milk crates, I have managed to put together a stack of serviceable bookshelves out of milk crates held together with zip ties.  My stack is about 5 1/2 feet high.

However, I used heavy duty milk crates, the kind that actually carry milk.  I'm not sure how well this would work with the "milk crates" sold in stores.
